Follow up with appointments every six months or sooner.
You'll be rewarded with Restylane Rewards® Benefit Checks of increasing value, after your first treatment and each of the next four treatments. After your fifth treatment, you will be rewarded with a $200 treatment voucher towards your next Restylane treatment.
To participate, you'll need to submit a copy of your physician's invoice, plus the end flap from the Restylane box(es) that were used in your treatment. You'll also need to register on this website and then fill out and print a Restylane Rewards Redemption Form.

Introducing Restylane Rewards® Premium Status –
Earn more after your fifth treatment
After your fifth treatment as a Restylane Rewards® member you earn Premium Status. As a Premium Status member, rewards are based on how much Restylane® you use in your treatments. You also continue to be rewarded for the number of Restylane treatments you receive. That means Restylane customers who have follow-up treatments have the opportunity to accumulate more rewards.
The long-term safety and effectiveness of Restylane, beyond one year, has not been investigated in controlled clinical trials.
As a Premium Status member, you will get a $200 treatment voucher on a future Restylane treatment from any participating provider after you use 4.0 mL of product in your treatments (click here for examples). You'll also receive a $25 Restylane Rewards Benefit Check after every qualifying Restylane treatment you receive. (A qualifying treatment must use at least one 1.0 mL or two 0.4 mL syringes.)
To receive your Restylane Rewards, simply click here to register for Restylane Rewards. When your treatment has been received, complete and print the Restylane Rewards Redemption Form and mail it along with your receipt documenting treatment with Restylane (credit card receipts cannot be accepted as proof of treatment) and the end flap from the Restylane box(es)** containing the Medicis Aesthetics™ logo (click here for examples) to:

To qualify for a reward from Restylane Rewards® each treatment must use a minimum of 0.8mL of Restylane® in any combination of syringe sizes (at least one 1.0 mL or two 0.4 mL syringes). Maximum of three treatments in the first 12-month period are eligible for Restylane Rewards. To qualify, treatments must be completed within six months of last treatment; 30-day minimum allowable time between qualifying treatments as a Premium Status member.
